(How we keep information about you safe)
At CHYPS Plus, we want you to feel free to talk about your problems without any worries. You can be sure that anything you discuss with any member of CHYPS Plus: doctor, nurse, development worker, psychologist, receptionist etc. will stay within this team. This means that you can tell others about this visit, but we won’t.
Nothing will be said to anyone – including parents, other family members, care workers or tutors – without your permission.
This applies even if you are under 16 years. On rare occasions, we might have to consider passing on information without permission. This would be to protect you or someone else from serious harm. We would always try to discuss this with you first. Sometimes it is useful to involve other people in your care.
For example if you are being treated elsewhere such as at the hospital, family doctor or a Brook clinic, we need to share information so we can give you the best possible care. This will benefit both you and us. Because of this, we may ask your permission to share information with other health professionals.
